On November 11, the 2023 Shanghai Medical Trade Fair—the seventh National Medical Trade Fair—was grandly held in Shanghai. Co-hosted by Kan Yi Jie Media and the Institute of Social Medical Institutions at Shanghai Jiao Tong University, and supported by the China Doctors Group Alliance, the Shanghai Municipal Association of Social Medical Institutions, the Jiangxi Provincial Association of Privately Operated Medical Institutions, and other organizations, the event brought together over a thousand distinguished experts and scholars from across the country, along with leaders from health authorities, hospital administrators, founders of physician groups, and heads of healthcare‑empowerment organizations, who shared their insights and engaged in insightful discussions.

Event Background

　　As a tertiary-level rehabilitation hospital, Nanjing Zijin Hospital primarily treats patients with severe traumatic brain injuries. Unlike patients with other conditions, their discharge does not signify full recovery; rather, it marks the beginning of a new phase—home-based rehabilitation.

　　Hospital medical staff and rehabilitation professionals frequently receive requests for assistance from patients who have been discharged and are recovering at home. They respond to each inquiry individually online; however, they have also observed that online communication can sometimes make it difficult to accurately assess a patient’s limb‑function status, while patients may struggle to fully understand and implement the prescribed rehabilitation plan.

　　Following deliberation, the hospital’s leadership decided to further enhance its rehabilitation services. Beginning in March 2023, the hospital formulated and launched a free, at‑home rehabilitation guidance and follow-up program rooted in “heart‑centered” care. This initiative underscores Nanjing Zijin Hospital’s steadfast commitment to patient‑centricity and represents another constructive effort to innovate and improve patient satisfaction.

Activity Implementation

　　“Starting from the Heart—Heartfelt Follow-Ups, Compassionate Medical Support” has, since its launch, conducted several rounds of follow-up activities, serving home‑based rehabilitation patients both within and outside Nanjing. The program’s reach continues to expand, and it has assembled a well‑structured, multidisciplinary rehabilitation team—including physicians, nurses, occupational therapists, physical therapists, and speech therapists—that provides tailored, on‑site guidance for patients with diverse functional impairments, thereby maximizing support for their home‑based recovery needs.

　　At present, the “Starting from the Heart—Heartfelt Follow-ups, Close‑by Medical Support” follow-up campaign not only boasts a robust team of experts but has also established a comprehensive follow-up system.

　　Because patients undergoing home-based rehabilitation vary in their conditions, limb-function status, living environments, caregiving arrangements, and psychological states, Nanjing Zijin Hospital adopts a multi‑pronged approach to its in‑home follow-up visits. The team provides guidance on family‑environment assessments, rehabilitation physical examinations, the development of personalized rehabilitation plans, medication management, and psychological support, thereby addressing the challenges patients face and preventing potential safety risks.

　　Looking back on these follow-up cases, we’ve gained so much: we’ve met patients’ eyes brimming with hope, held the firm, reassuring hands of their families, and experienced countless moments of warmth and heartfelt emotion. The comments beneath our repeatedly shared follow-up video reports are a constant source of motivation for our outreach team.
